When COVID forced services to shut down, a small team in Saskatchewan’s southwest stepped in to make sure vulnerable patients weren’t left behind. That emergency “transition team” quickly grew into a powerful model of connected care – blending crisis response, patient navigation, and assertive outreach to guide people through complex mental health and addictions needs.

Four years later, these teams are now thriving across the region.

This QI Power Hour session shares the story of how a small idea became an essential system of care – told by the leaders who built it. You’ll learn how the model works, the impact it’s having on patients and providers, and why it’s acting as a blueprint for future mental health and addictions services in Saskatchewan.

Speakers

Michael Seiferling is the Director of Mental Health and Addictions Services Southwest for the Saskatchewan Health Authority. He has been supporting complex mental health and addiction services for over 25 years. Starting in 2001, he has worked in both inpatient and outpatient programs which have focused on addictions, serious and persistence mental health, and acquired brain injury. He is passionate about providing trauma informed services for both staff and patients and holds deeply rooted respect for patient experience.
