For over 20 years, HQC has played a leading role in building capacity for quality improvement, bringing together key partners, and supporting our health care system to think and act as one. As we launch our 2024-25 annual report, we celebrate all the incredible work our team and partners have accomplished together.
Continuous improvement is who we are. It shapes how we show up as an organization, grow as one, and measure success. Over the past year we have worked hard to address issues facing our provincial health care system, optimizing health care one process improvement at a time. In 2024-25, we brought our new five-year strategic plan to life, building on our longstanding commitment to strengthening our health system’s capacity for quality improvement, with a special emphasis on promoting large-scale change in our strategic focus area:
Key milestones to celebrate from 2024-25:
- 5 new partnerships with First Nations and Métis partners in collaboration with Four Winds’ life promotion efforts
- 39 participants from primary care clinics completed the four-month QI in Clinics training program, focused primarily on improving access to care
- 119 members joined the Thrive at Home Action Community, co-creating a future where everyone can grow older with dignity, choice, safety, and a true sense of belonging
- 63 on-going research projects, 10 of which were initiated this year, leveraging our analytical expertise in collaboration with health system partners
- 1,336 registrants of QI Power Hour, our free monthly webinar series sharing knowledge, best practices, and innovative ideas related to quality improvement
- 2 provincial accountabilities as a result of the relationships we’ve built and the impact we’ve demonstrated as leaders in quality improvement for the past 23 years
Not unlike years past, collaboration remains at the heart of our work. Whether partnering with communities, healthcare providers, or system leaders, we know that real change happens when we come together around a common vision. Over the past year, we used this spirit of partnership to co-create solutions that are both innovative and grounded in lived-experience. This momentum allowed us to take major strides within our Thrive at Home community — an initiative focused on understanding and addressing the complexities facing older adults. Our Action Community gained 119 new members over the course of the year, who have come together to improve transportation, navigation, and home supports to support healthy aging. The success of these networks has led to insightful system mapping and learning circles that shine a light on the promising initiatives happening in this area across the province. Capacity building is another cornerstone of our culture. We are deeply invested in strengthening skills and knowledge across the system, ensuring that improvement isn’t siloed to one team or project, but is embedded and sustainable province wide. This past year, HQC played a pivotal role in working with primary care providers to drive the emergence and integration of team-based care in the province. We have set ourselves up to work closely with primary care teams over the next year, helping them craft quality improvement plans for their unique clinics – instrumental work to ensuring their success in delivering better care for communities. The details of this work and more on where it’s headed can be found on pages 17-19 of our annual report.
And at the backbone of everything we have accomplished over the past year, we have relied on research, evidence, and best practices to guide our decisions. By integrating high-quality data, evaluation, and continuous learning in our efforts, we continuing building the road for measurable and impactful change.
These incredible achievements would not be possible without the dedication, support, and drive of our health system and community partners, Board of Directors, and the team at HQC.
The second year of our strategic approach will bring fresh opportunities — and no doubt, new challenges — but many hands make light work. We will continue pushing for bold, lasting impact for care in our province, while adapting and evolving to emerging priorities.
We enter the year with a renewed sense of purpose, along with clear, actionable goals. Remaining dedicated to collaboration and innovation, we will focus on deepening partnership, scaling successful initiatives, and ensuring the strides we make today pave the way for sustainable, long-term impact tomorrow.
