This report outlines the approach and results of the Chronic Disease Management Collaborative (2005-2008), focused on improving care and outcomes for those living with diabetes and chronic artery disease (CAD) . At the time, it was amongst the largest quality improvement initiatives in Canada, and the largest improvement initiative in Saskatchewan, engaging 33 family physician practices and approximately 15,000 people living with diabetes and CAD.
