For her CQIP project, Dr. Amanda Ellwood looked at discharge prescription from discharge from hospital and medication reconciliation. The project, which was done in partnership with Dr. Ron Taylor worked to look at reducing medication discrepancy at discharge by 60% by Sept. 1, 2021.
