Session description
Healthcare systems across Canada continue to face complex challenges related to capacity, access, demand, and workforce pressures. Rather than pursuing incremental improvements, Southlake Health, which serves a 1,200 square kilometre catchment area north of Toronto, Ontario, has embarked on a bold transformation guided by the Quintuple Aim — a framework focused on improving patient experience, population health, provider well-being, health equity, and system value.
Join us on July 17 from 9 a.m – 10 a.m. CST to hear how Southlake is reimagining healthcare delivery through a Distributed Health Network, population health management, and a system-wide approach to improving outcomes for patients, providers, and communities.

Guest Speakers
Dr. Paul Woods, MD, President & CEO, Southlake Health
Dr. Paul Woods is a senior healthcare leader with a wide range of clinical and executive experience in both Canada and the United States, highly regarded for his proven ability to lead positive change in complex environments. Paul is well known for his improvement work at London Health Sciences Centre, Trinity, Health, and Alberta Health Services. Originally from Ontario, Paul obtained his medical degree from Western University and his Masters from the Institute for Health Policy and Clinical Practice at Dartmouth College.
Tyler Chalk, Executive Vice President & Chief Strategy Officer, Southlake Health
Before Southlake Health, Tyler Chalk served as Director of Strategy at SickKids and in leadership roles at Mount Sinai Hospital in Toronto and at the Ontario Ministry of Health. He has contributed to published research on the healthcare funding reform and has spoken at national and international conferences on data-driven outcomes improvement and the shift to value-based care. Tyler has an undergraduate degree in economics from McMaster University and did his Masters at the London School of Economics.
Matthew Meyer, PhD, Executive Director of Population Health, Southlake Health
Matthew Meyer is the Executive Director of Population Health Management at Southlake Health and an Adjunct Assistant Professor in the Department of Epidemiology and Biostatistics, Schulich School of Medicine and Dentistry, Western University. He is a Vanier Scholar, who’s professional and academic careers have focused on the implementation of Population Health Management principles for health system transformation. He has served as an advisor to numerous national, provincial, and regional healthcare agencies on research, evaluation and system transformation.
