Project: Reducing length of stay at Saskatchewan’s Psychiatric Rehab Hospital

For her CQIP project, Amy Soubolsky looked at how to reduce unnecessary time spent in the hospital for patients, and to improve the experiences of patients and the outcome relating to their discharge. The project, which was done in partnership with Dr. Adewale Adeoye and Nurse Practitioner Philip McGee focused on creating a discharge readiness checklist and improve the discharge assessment process. They worked to improve seamless care and reduce inefficiencies in the discharge process, hoping to reduce the length of stay.
